Box 2 reads: "Fabian's Pharmacy, M. D. Weagley, P. D., N. H. Shull, M. S., Radcliffe and Mulberry Streets, Bristol, PA. Phone 8-2444" and "683839, 2-3-54 [date], Mary Jane Mannberry. One tablet three times daily. Dr. McIlvaine."
